Ultra-targeted accelerated partial breast irradiation using TARGIT – a cautionary note

چکیده

Accelerated partial breast irradiation (APBI) is an abbreviated radiotherapy course delivered to the tissue surrounding the excision cavity. It is under intense clinical investigation as a therapeutic approach for low-risk early-stage breast cancers. Phase II studies of APBI using multicatheter brachytherapy or externalbeam radiotherapy show high local-control rates at 7–12 years median follow up. These studies highlight the long natural history of the low-risk early-stage breast cancers for which APBI is being proposed. It therefore seemed prudent to wait for the 5–10 year follow-up data on ~16,000 women enrolled in seven randomised controlled trials comparing APBI with whole-breast irradiation to make definitive conclusions on the safety and efficacy of APBI and to establish the clinical, pathological or technical contexts where caution should be exercised with different APBI techniques. However, the investigators of one of these trials, the TARGIT trial, thought otherwise. In their report, the TARGIT investigators state that their trial “provides robust andmature evidence... showing that targeted intraoperative radiotherapy is safe,” and concluded that, “for

Commentary on "Accelerated partial breast irradiation consensus statement: Update of an ASTRO Evidence-Based Consensus Statement".

5 Although the new American Society for Radiation Oncology (ASTRO) consensus statement on accelerated partial breast irradiation (APBI) reflects many important changes relative to case selection and inclusion criteria for APBI, we would like to address our concerns specifically regarding the recommendations on the use of low-energy x-ray intraoperative radiation therapy (IORT). The consensus sh...

A cohort analysis to identify eligible patients for intraoperative radiotherapy (IORT) of early breast cancer

BACKGROUND Since the results from the randomized TARGIT A trial were published, intraoperative radiotherapy (IORT) is used more often. IORT can be provided as accelerated partial breast irradiation (APBI) or as a boost. The definition of suitable patients for IORT as APBI differs between different national societies (e.g. ESTRO and ASTRO) and different inclusion criteria of trials and so does t...

Accelerated partial breast irradiation: a review and description of an early North American surgical experience with the intrabeam delivery system.

BACKGROUND Targeted intraoperative radiation therapy (IORT) as an alternative to whole breast irradiation (WBI) has been described for patients with early-stage breast cancer. The randomized phase III TARGiT trial demonstrated similar recurrence rates to WBI and a lower overall toxicity profile on short-term follow-up. We report on our early North American surgical experience using the Intrabea...

Outcomes after accelerated partial breast irradiation in patients with ASTRO consensus statement cautionary features.

PURPOSE To evaluate outcomes among women with American Society for Radiation Oncology (ASTRO) consensus statement cautionary features treated with brachytherapy-based accelerated partial breast irradiation (APBI). METHODS AND MATERIALS Between March 2001 and June 2006, 322 consecutive patients were treated with high-dose-rate (HDR) APBI at the University of Wisconsin. A total of 136 patients ...

Is IORT ready for roll-out?

Two large randomised controlled trials of intraoperative radiotherapy (IORT) in breast-conserving surgery (TARGIT-A and ELIOT) have been published 14 years after their launch. Neither the TARGIT-A trial nor the ELIOT trial results have changed the current clinical practice for the use of IORT. The in-breast local recurrence rate (LRR) after IORT met the pre-specified non-inferiority margins in ...
